Job Qualifications: Minimum Experience: 6 months related exp pref Required License/Certifications: CA C.N.A., BLS, MAB (90 days upon hire). Job Summary: Involves, informs & supports the pt./family in the overall care of the pt. Makes frequent rounds, answers call lights. Reports changes in condition/behavior of pt.: Recognizes age-specific needs/assist in the care with other caregivers. Prepares pt. For meals, assist in feeding, records I&O: Prepares room for admission/transfer/discharge. Transports pts. Does VS, Ht.&Wt accurately. Secures pt belongings according to P&P: collects/labels specimens per P&P. Performs pt/family teaching or core education: Assists RN/LVN in attempts to overcome barriers to learning of pt. Provides environment conducive to safety & security & implements appropriate precautions/measures. Aware of pt limitations & delivers care appropriately.: Assures proper ID, allergy & Fall bands on pts. Keeps safe/clean environment, reports defective equipment when seen: Maintains fall precautions on pts. At risk. Communicates with nurse re: pt status: Assists in orientation of new team members (CNA's). Identifies/responds to critical changes in pt. & reports to RN. Knows role in a code situation. Has skill in the use of equipment & devices pertinent to the care of the pt. Provides skin care. Repositions pt for maximum comfort & hygiene measures. Gives general hygiene to patients. Upholds basic rights of privacy & seeks to preserve the dignity/independence of expression of all pts/families Maintains/updates own knowledge & competencies. Practices appropriate measures of infection control according to principles of standard precautions. Obtains patient's self report of pain when taking vital signs & reports to nurse. Observes pt for signs of discomfort.
