Experienced Remote Social Worker – Medical Case Management and Community Resource Coordination

Job Summary We're seeking an experienced remote social worker to join our team as a Medical Case Manager and Community Resource Coordinator. This role will partner with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. As a remote social worker, you'll work with members to research available options and resources for financial, transportation, safety, and housing needs. This position requires an LSW/MSW with an unrestricted active license and 2+ years of experience in medical social work or case management.

Responsibilities

As a remote social worker at careerzynith, your key responsibilities will include

• Collaborating with a registered nurse case manager to assess, plan, and advocate for members' benefit plan and/or health needs
• Researching available options and resources for members with financial, transportation, safety, and housing needs
• Empowering members to make positive lifestyle and medical choices
• Working as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
• Conducting telephonic or electronic exchanges with members to provide support and guidance
• Maintaining accurate and up-to-date records of member interactions and outcomes Requirements To be successful in this role, you'll need
• An active and unrestricted LSW - Licensed Social Worker, LMSW - Licensed Masters Social Worker, LCSW - Licensed Clinical Social Worker, or LISW - Licensed Independent Social Worker license in the state of residence
• 2+ years of experience in medical social work or case management
• A Master's Degree in Social Work
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong problem-solving and critical thinking skills
