Telemedicine Advanced Practice Provider (NP/PA) – Skilled Nursing & Post-Acute Care Position Overview We are building a collaborative team of Nurse Practitioners and Physician Assistants passionate about delivering high-quality overnight care to skilled nursing and post-acute patients through telemedicine. This role is ideal for clinicians who enjoy acute clinical decision-making, autonomy, and meaningful collaboration with bedside nursing teams while helping support safe treat-in-place care and appropriate escalation when needed. Providers care for a geriatric and post-acute population with varying degrees of illness severity, supported by structured protocols, ongoing mentorship, and a team-oriented telemedicine environment designed to help clinicians grow in confidence and independence over time. Whether you are an experienced clinician or a motivated new provider interested in post-acute medicine, we are committed to creating a supportive onboarding and training experience that helps providers succeed in telemedicine care. Clinical Scope & Patient Population Providers care for a diverse geriatric and post-acute skilled nursing facility (SNF) patient population with a focus on:

  • Responding to urgent and acute calls from skilled nursing facilities
  • Evaluating and managing common SNF clinical concerns via telemedicine
  • Providing clinical recommendations, treatment plans, and escalation guidance
  • Determining when ER transfer or higher level of care is appropriate
  • Documenting encounters accurately and efficiently within the EHR
  • Collaborating professionally with nursing staff, facility providers, and telehealth leadership
  • Following clinical protocols and workflows while utilizing independent clinical judgment
  • Participating in onboarding, training, and occasional team meetings

Providers primarily deliver overnight telemedicine coverage (12-hour shifts, 7p–7a) for skilled nursing and post-acute facilities. Providers are expected to evaluate and manage acute change-in-condition scenarios through protocol-driven telemedicine assessments and clinical decision-making. Providers must be comfortable independently managing acute overnight clinical concerns in the SNF setting using telemedicine workflows, nursing collaboration, and established escalation protocols. Common Overnight Clinical Scenarios Include:

  • Altered mental status and delirium
  • Fever and infection concerns
  • Respiratory symptoms
  • Falls and injury assessment
  • Hypertension and acute vital sign abnormalities
  • Dehydration and poor oral intake
  • Chest pain and cardiovascular concerns
  • Urinary symptoms and possible UTI evaluation

  • Active Kentucky (KY) license required or willingness to obtain
  • Tennessee and Florida (FL) license preferred or willingness to obtain
  • Open to obtaining additional state licenses as program expands

Scheduling Philosophy We believe flexibility and sustainability matter. Our scheduling model is designed to support both excellent patient care and long-term provider wellness. We work collaboratively with providers to balance coverage needs while maintaining scheduling flexibility whenever possible. Scheduling Expectations Currently Include:

  • Minimum commitment: 24 hours/week (2 shifts)
  • Hours: 7p – 7a
  • Primarily overnight 12-hour shifts
  • One weekend shift monthly
  • Participation in one major or minor holiday annually with advanced scheduling and collaborative planning
  • Opportunities to pick up additional shifts as the service line grows
